Our model of care is embedded on three core values; ASSIST, ENGAGE, REFLECT. These values form our culture and practice of delivering excellent care and support to people within our care. The Grant Healthcare model of care is person-centered driven and well-coordinated to meet the needs of our service users. This is a model of care that is integrated for the benefit of our service users. Our model is designed in a holistic way that caters;

The model is flexible in meeting the needs of service users in order for them to gain independence. The needs of care are different for each and every person hence there is need for strong care and support planning processes.

At Grant Healthcare we believe care is universal, it revolves with time due to differential factors, hence our model is circular and continuous. These three processes are essential towards quality care delivery.

Person Centered Care

Grant Healthcare delivers person centred care to individuals within our care so that the care they receive is right, consistent and effective. We believe in dignity, compassion, respect, coordination and personalization.

Individuals within our care are given choices of what they want such as; choosing what to wear or eat for the day.

Our person-centered care involves;

COMPASSION

Our care is based on strong relationships which focus on dignity, respect and empathy.

Compassionate care is an integral part of achieving effective patient outcomes through emotional support and confidence. Providing compassionate care is part of Grant Healthcare DNA and fosters a positive patient’s emotional state.

Compassionate care reduces depression and stress on patients and may impact positively towards recovery time and satisfaction.

Our staff are equipped with skills that are essential towards compassionate care;

Emotional Intelligence- Our staff strive to provide high emotional intelligence on handling interpersonal relationships with patients and other staff.

Confidence-Our staff are confident in communicating and delivering care to patients with a clear vision of fostering a caring culture built on trust, integrity and honesty.

Critical thinking-Grant Healthcare staff provide compassionate care through high standards of critical thinking and problem solving.

Cultural awareness-Our organisation provides patient-centred care, thereby takes into consideration cultural awareness and implement it within our relationships.

Resilience-Our staff participate in reflective activities and get to understand the patients’ personal values and work together to increase hope and reduce stress.

Our staff are compassionate and empathetic towards the pain and suffering of our patients hence they ensure the patients are comfortable, free from emotional stress and are given enough support and confidence during their treatment.

COMPETENCE

Grant Healthcare provides effective care through competent and well-trained staff. Our staff have clinical and technical expertise which enables them to build strong relationships with individuals within our care. We provide specific training to our staff to enable them to effectively deliver care to individuals in our care. We train staff in courses such as;

Mental Disorders:

Neuroses such as depression, anxiety disorders, obsessive-compulsory disorder (OCD), Phobias and Post Traumatic Disorders (PTSD)

Psychosis such as schizophrenia, bipolar (manic depression) Personality disorders (we assist individuals with depression and substance abuse and aim for well-being and independence)

 

COMMUNICATION

Communication is vital for successful care relationships and is effective for teamwork and has a lot of benefits towards individuals in our care.

Effective communication enables us to deliver high quality person-centered care.

Our staff have mastered different types of communication including sign language, Makaton or Braille.

Communication enables our staff to uphold privacy, dignity and promote independence to individuals within our care.

To our service users, good communication ensures that their needs and wishes are met and avoid miscommunications and misunderstandings. This will ensure confidence and trust.

Our staff are trained on how to keep service users’ information and communication confidentiality, thereby respecting individuals personal data.

COURAGE

We have the courage to do good to the people in our care through innovation and embrace new better ways of caring.

Our organisational values are centred on doing the right thing, highlighting any issues or concerns so that we have strength and a clear vision in providing quality care.

Our courage enables us to put our patients first and deliver quality care they deserve.

Our dedicated staff possess skills in different forms of courage including; physical, social, moral, emotional, intellectual and spiritual.

Our staff take responsibility for the care that they provide, ensure safety and make judgements that affect people in our care.

Staff have the courage to speak out when they notice that certain things can be done in a more effective way in order to improve the care they provide.

We are committed to protect young people and adults in our care to live safely, free from abuse and neglect. We protect our service users’ health, well-being and human rights.

Safeguarding is important to us because;

We are also committed in full to The Care Act 2014 and adhere to the principles of Prevention, Empowerment, Protection, Proportionality, Partnership and Accountability.

Grant Healthcare is also committed to the Mental Capacity Act 2005; to protect and empower people in our care who may lack mental capacity to make decisions about their care and treatment.

We are committed to safeguard all forms of abuse to people in our care. This may be; Domestic, Emotional, Psychological, Modern Slavery, Physical, Institutional, Discriminatory, Financial, Sexual, Self-neglect or Neglect.

All signs and indicators of abuse such as;

Assault, physical punishments, involuntary isolation, misuse of medication, inappropriate restraints, force feeding and rough handling of individuals are taken seriously and follow our safeguarding policy.

Our staff are trained to be responsible on all safeguarding matters and are committed to ensure that the child/adult within their care are safe from any danger and support them as much as possible.

ENGAGE

At Grant Healthcare we believe that service users should actively participate in shaping their personal health plan, based on their knowledge and experiences of what works best for them.

We ensure service users’ voices are heard and that they can improve services through their influence on local and national policy. Service user involvement can be therapeutic and can help to increase confidence, raise self-esteem and develop new skills. Service users’ involvement emphasises the importance of human rights, equalities and social models of disability.

We believe service users should be empowered in managing their own health, care, treatment and support in order to improve their health and wellbeing and at the same time giving them the best opportunity to lead a desirable life.

We incorporate the NHS values;

Independence is about giving service users control of their care and support and be able to meet personalised services that have desired outcomes in a holistic manner.

Our aim is to ensure that our services are flexible and of good quality. We can only meet these needs through effective engagement with our service users. Service users’ input is essential to provision of quality and responsive services.

We use the SCI Model of Co-production as a spectrum of engagement where;

Engagement through input involves making the existing commissioning and procurement practices open for collaboration, gathering

input to affect services through the use of questionnaires to service users, carers and providers asking for comments on needs or services, reviews of service users suggestions in regard to the planning and design of services.

The service users should be empowered to be involved in making sure that mental health services, organisations and policies are led and shaped through their experience and knowledge.

Service users should be involved in planning , development and delivery of services because they have unique experiences, skills and abilities that enable them to provide advice. For example substance misuse treatment is more effective if developed and delivered by people directly involved in their use.

Therapeutic engagement

Our aim is to support the services within our care to stay well and manage their own conditions better so they can lead a healthy lifestyle.

At Grant Healthcare therapeutic engagement enhance sense of well-being and promote physical, cognitive and emotional health of our service users.

A balanced daily routine of a person’s life includes the productive, leisure and self-care.

At Grant Healthcare we have people in our care who have experienced hurt, social and relationship struggles and therapeutic engagement with them forms a new template in their lives on how they view other people. It is our focus that strong therapeutic engagement brings positive relationships.

A strong therapeutic relationship enables a service user an opportunity for someone to listen, absorb their feelings before being reflected back.

At Grant Healthcare we believe strong therapeutic relationships enable confidence, reassurance, openness and honesty. It also allows legal, ethical and professional processes to be adhered to.

Therapeutic relationships ensures service users are able to open up more on their needs, physical, mental and emotional well-being. The treatment process becomes more positive as service users are able to share how effective treatments are, their side effects and a variety of other issues.

At Grant Healthcare reflection is a process in which we gain insight into our professional practice through analytical thinking. Reflection is of paramount importance in our organisation because it has positive impact and is good practice.
